The traditional model for risk assessment focuses on Probability and Severity. While effective for mechanical or electrical risks, it becomes more complex when people are involved, adding a 3rd Dimension. Competence can lead to complacency, resulting in “Mind Not On Task,” which explains why many serious injuries occur among experienced workers.
Human Factors such as rushing, frustration, and fatigue further impact safety, efficiency, and decision-making. In addition, since over 90% of fatal workplace injuries involve mechanical or kinetic energy, the ability to react in time (“eyes on task”) becomes critical, yet often overlooked in many SIF prevention programs.
In this session, participants will learn the key causes of SIF’s, the role of inattention and complacency, and how to better assess risk by incorporating human factors and situational context. A practical case study, revised risk assessment/JHA, and simple engagement tools for supervisors will also be shared.
